U.S. Highway 65 in central Arkansas enters Faulkner County at Damascus in the north, runs together with Interstate 40 from Conway to Little Rock, and then runs with Interstate 30 and Interstate 530 down to Pine Bluff.

The highway has been relocated from its earlier path which is now marked as Arkansas Highway 365 between Conway and Pine Bluff. The highway ran south out of Conway following Harkrider Avenue, parallelling the Union Pacific Railroad line on the est side of the Arkansas River. This is sometimes called the Old Conway Highway. In North Little Rock the highway followed what is now designated as Percy Machin Drive, turning east onto Eighteenth Street. The highway followed Maple Street and Main Street south at Eighteenth Street's intersection with these roads. Northbound traffic on the highway used the one-way Main Street, southbound traffic passed down Maple Street. The highway crossed the river at the Main Street Bridge. In downtown Little Rock northbound traffic followed Scott Street and southbound traffic followed Main Street until the intersection with Roosevelt Road was reached. At this point the highway ran along Roosevelt east before turning south again onto Confederate Boulevard on the southeast side of the city.

The highway enters the state of Arkansas in the north approximately twenty miles north of Harrison and exits in the south in the extreme southeast corner near Eudora.
